This particular error historical comes from Forge and Optifine incompatibilities, I'd try Forge 10.13.4.1558 and Optifine see what happens. If that doesn't work try rolling optifine back a version or so too.
Graphics driver issue looks like, whats your specs such as MC Version, Forge, any mods, lastest log would be handy, and your pc specs like cpu, gpu, driver versions for gpu. You can get all the non Minecraft info from running dxDiag in Run.
Welcome to Minecraft 1.10.2, Try upping your memory allocation to 6g. I run a 1.10.2 Modpack with 187 mods with shaders and a resource pack it will not run with anything less than 8g but prefers 10g.
Try downgrading to Java 8u51 you can get that from Oracle, Some older Intel chipsets don't play nice with Minecraft on Windows 10 and Mojang suggests using this version of Java as a potential fix. As well I notice you have a very small amount of memory allocated to Minecraft, you may also try giving it more RAM.

I'd try giving Minecraft some more memory, "Caught exception from Pixelmon (pixelmon)Caused by: java.lang.OutOfMemoryError".

Oh the ole
EXCEPTION_ACCESS_VIOLATION (0xc0000005)
bug eh?
Generally this is caused by 3 things;
1. Not enough memory allocated to minecraft.
2. Corrupted or simply outdated Java
3. (Most Likely Candidate) Incompatible graphics drivers as posted here in the FAQ for this forum.

Yup that would be from Actually Additions, it's the "Traveler's Sack".
