So I finally know the problem. I think its because of the performance. I turnd of the Texture packs, switched to Fast (not fancy) and the render distance is at Tiny. Now I can create a 256 Map. But all above kills the game.
But its funny.
My System:
Intel Core i5 3.1GHz
8 GB Ram
1024GB VRAM
I can't understand why this wont be able to play....XD

I can't figure it out, I just downloaded it fresh, downloaded a new minecraft.jar, installed the mod, and it ran perfectly at 256 height.
I don't know what to say, I've done the same, new install of minecraft, redownloaded 1.9-3 minecraft.jar, redownloaded heightmod.zip, Deleted META-INF, inserted the class's and it still crashes. All thats changed is 2 numbers on the crash report.
Also, your links are confusing. In the height mod section you have a .zip called heightmod.zip for the SP/GUI one, but in the direct links underneath everything you have a heightmodpre3.zip under 'height mod' that doesn't give a GUI in the map creation screen even though it has the .class's for it
EDIT:Nvm, I was putting the heightmodpre3 into a 1.8.1 minecraft.jar :tongue.gif: The GUI does work and it crashed just like the other one
Guys, I've tested this multiple times, and cannot reproduce it, in a dev enviroment or my client. I really can't fix it if I can't produce it.
EDIT:Referring to the Height Mod bug, remember, it's for 1.9pre3, maybe the wrong version?
Could you explain your exact process for installing this?

Installation
Windows:
1) Open up %appdata%, if you don't know how to do this, start>run, then type in %appdata%
2) Browse to .minecraft/bin
3) Open up minecraft.jar with WinRAR or 7zip.
4) Drag and drop the necessary files into the jar.
5) Delete the META-INF folder in the jar.
6) Run Minecraft, enjoy!
Because I'm doing exactly that, i'll try reinstalling Java to see if that has any affect.

Define 'map rendering program' because the latest test build of MCEDIT works with map heights of 256

No luck, still crashing. I've tried different names, different seeds, different modes, nothings working.

Well i'm using Java 6 update 27. Also, when i go to make a SP map 512 blocks high it freezes a couple of seconds into "building terrain"
