I'm not sure what the number seed is, but the text I entered for the seed was "Cottonmouth".

My world that I created after the latest update seems to be completely missing Nether Fortresses. I know it's possible for a world to generate without one, but that's never happened to me before in the dozens of worlds I've played on. I went into creative and flew around to double check but could not find a single fortress. Oddly enough though, I did find Wither skeletons clustered on a patch of Netherrack. My understanding is that they only spawn inside the bounding box of a fortress. I did explore that area pretty thoroughly, digging down and such, but there's definitely no fortress there.
Checking the official bug listing, I've not seen this particular issue listed yet. Has anyone else ran into this, and if so, is there a way to fix it?
I can deal with some bugs, but this one is pretty much a game breaker for me.
PS. Sorry, I just realized I posted this in the Xbone section and i'm on a 360.

Just thought this might make a fun question. So how do YOU do it?

Wow. Got way more response than I expected. Thanks to all for the great information. Another question, occasionally I will find a golem on top of the glass on the upper platform. I'm not sure how it gets up there since it shouldn't be able to spawn there and afaik golems can't jump. Would I be better off lining the top with half slabs instead, or maybe slabs and glass?

Thanks for your response. If I am understanding you correctly, your design is based on Nims with a few tweaks. You've made the floors 16 x 16 instead of 10 x 10, with a 2 x 2 space filled in at each corner so the water flows correctly. You also use 3 high spacing, and separate the villagers into two pods on opposite ends of the farm? These all seem like very easy changes to make, especially if it triples the efficacy of the farm.
Do you use more doors or is 40 still sufficient? Also, how many villagers are needed for golems to spawn? I thought it was 14 but I could be wrong. I am playing on console and with the last update breeding behavior was changed to be the same as 1.8 PC, so i'll need to make a breeder and then move them in one at a time which is a pita. Don't want to bother with more noses than I absolutely have to.

I've always used NimsTv iron farm in my SMP worlds. I find it to be a good design for output given Vs. cost of materials and has always provided enough iron for me.
Here's the video in case you're not familiar with it:
I've always built it exactly as described, but I have found the item filter to be unnecessary and have opted for four hoppers at ground level feeding into a double chest instead. My question is, without the item filter is it still necessary to build it at a height of ten? It seems that if he drop shaft were at least three high to accommodate the golems it should function just as well, while saving on resources (and making it easier to get the villagers into it)
Should this work, or am I overlooking something?

This is a well known issue caused by server lag. That's why it only occurs when you' playing solo survival and not on multiplayer. A few things to try: Make sure you have the most up-to-date Java installed, turn down graphics, and try optifine plugin.

Unfortunately this extremely common in my experience. The game requires a random amount of bonemeal to grow a tree, especially when it is being fired from a dispenser. It got to the point that it was taking me longer to gather the bonemeal to feed my farm than it would to just plant the trees and let them sprout naturally.
The solution i found was to start using spruce. I'll describe the farm I use if you want to try it out for yourself. It is a manual farm, but it only takes 4 to 5 bonemeal to yield about 1 to stack of logs (or 4 stacks of planks). Well worth it.
You will need an 8 x 10 area with nothing overhead. Dig out 9 rows 8 blocks wide by 1 block deep, then dig out a 10th row 8 wide by 2 deep. Place a row of 8 dispensers with water buckets at the end opposite the 2 deep row and wire them however you want, just as long as you can turn them on and off.
Now in the 2 deep row pick a side and dig over 1 and down by 2 blocks. Place a chest with a hopper feeding into it to collect your saplings. Place a bucket of water in the other corner. Next, mark off a 2 x 2 area in the center of the pit and plant your 4 spruce saplings there. Bonemeal them until they grow into a mega spruce. Farm your wood however you like. Once the wood is all harvested, turn on the water dispensers so all saplings that drop will be pushed to the hopper chest. And that's it. Simple and efficient. Once all the leaves have decayed and the saplings are in the chest just turn the water off, replant, and bonemeal.

1) are you building this in creative?
2) is there any particular reason you picked this design?
If you are building in creative, then by all means go ahead. But, if you are trying to build this in survival I would recommend this design:
It will work for Enderman. Just build the bottom chamber and all floors 3 high instead of 2. You will lose a floor in height due to the spacing, but it doesn't really effect the output. This should be comparable to Snocrashes design as far as output, and it doesn't clog.
I built this myself over an ocean at Y = 215 and let it run for over an hour with no issues at all. It's also a lot more resource friendly since you don't need to cover the entire thing in carpet.
Anyway, just my two cents.
Side note: I haven't played PC MC in a while (more of a console guy) but i've heard that shifting floor designs are broken in 1.9 and possibly before. I know it works in 1.8 but, if you're playing 1.9 or a snapshot of it this design probably won't work.

Since villagers now harvest crops, and need to be willing to breed I've had to go to a PC 1.8 infinite breeder design. It seems to work, but the farmer villager isn't breaking / replanting crops like on the PC edition. I know he had an empty inventory when I spawned him, and I saw him throwing potatoes to other villagers to feed them (the only crop in my farm) so I know he had to have harvested them, but I didn't see him actually pick them up or replant them.
Am I missing something here, or does the farming mechanic work sans the harvesting / replanting animations?

I know the update had a ton of new features, but like with every update there are things not listed in the changelog. If anyone finds an undocumented feature, please share it here.
For instance, I discovered that mobs will now spawn above the nether roof. So for all of you that want an easy gold farm there you go.

I'm familiar with hopper clocks, but i've not played around with them too much. I'm working on a design that needs a row of dispensers to fire off every 19 minutes (to flush baby villagers out of an infinite breeder). Will any hopper clock work for this?
If I understand it right, the time interval is based on the number of items passing between the hoppers, so any idea how many items would set it for 19 minutes?

I would like to get some Ender pearls in order to find the End portal, i'm just not sure how to do it. I don't really like the idea of hunting for them. I've been playing on my current world for 20 or 30 hours and have only seen 2 Enderman total. Not really encouraging for hunting them. I thought making a farm for them, but designs for overworld Enderman farms are hard to come by and the ones I do find are massive builds that i'm not interested in.
Does anyone have any suggestions on how to get around 2 stacks of pearls without hunting for them or building a massive farm? I could always trade a villager for them, but getting enough raw materials for the trades seems even more tedious than hunting for them.
