I'm not sure what you're saying, but I modded a .jar file with edited textures so that the character and HUD was invisible, for the purposes of machinima.
I think "he" needs to be a mob that appears on every difficulty, on close fog, and just watches you. It would be kind of inappropriate to make what amounts to Notch's dead brother appear in Hell. It's just a little disturbing.
I routinely fall off my skybridge that goes out from my base. Also, when I perish upon hitting the ground, I spawn down in the valley that the bridge goes over. Which is the only place that mobs spawn in, pretty much.
