First issue is, nobody will use it on themselves because it's a harmful potion. However, nobody is going to use it on their opponents in PVP because it's PVP and nobody is mining while engaging in a fight. So not only is it useless in a PVP match, it would take up valuable time to throw it, when you could be attacking and causing real damage.

It also affects certain mobs. If a Creeper, Ghast, or a Wither has the Mining Fatigue effect, it's explosions will be less powerful and do less damage. Zombies with the effect will take longer to break down doors in Hard or Hardcore, and Endermen with the effect won't be able to pick up blocks.
