Download a shaderpack, open shader folder from shader pack selector, and drop shaderpack in there.
The mod by itself only allows shaders to work, but are not shaders themselves. This also explains the lack of images in the OP.

I use 4G myself for 32x/64x/128x RPs (I have to use 128x RPs without CTM myself else I get JVM errors which basically state I ran out of memory -- I cannot assign more memory to fix it -- MC just barfs and there doesn't appear to be anything I can do about it).
Maybe Synthesizer or someone with a Radeon 270/280/290 can pipe in if they have had any issues like this...
I'm here, so what do I need to test? (Have a 7970, but it's the same as the 280X)
Edit: Right now, I'm on Forge 1060 with shadersmod 2.3.12 and also on Optifine D1.
I installed forge using the auto installer and optifine by dragging it into mods. I will test the lastest forge and see if I get any errors.
Once upon a time, I got random crashes with java access exception violation errors, but I think the 14.4 drivers and other Optifine versions fixed it.
EDIT 2: Wait, wait, wait, wait.....
Did you try logging onto a server or joining a single player world? About two months ago, every time I joined a server, I got a crash log where the game exits in a bad state. I joined my SP world, which rarely crashes with that error, then the server, and the error is usually fixed.
Before I get the crash log, my MC froze up.
