Just my two cents here, I don't have a pony in this race. There isn't "people managing that code". There's only sp614x. He's doing this in his spare time, free of charge. One person, not people. He doesn't have to keep it up to date, he could abandon optifine at any time just like Khar did with MCPatcher.
You cannot compare one person working on a free mod in his spare time to teams of paid developers working on large projects like Direct3D or OpenGL. It's not even apples to oranges. It's apples to salmon.
Documentation is such a low priority, especially for those parts of the mod that maybe a dozen people in the entire world code against. ¯\_(ツ)_/¯

That requires you to have the Connected Textures Mod, or CTM, installed in your instance of Minecraft. The easiest way to do this, currently, is to install Optifine. Without CTM, the connected textures of any resource pack will be hidden.

It all depends on your resource pack. Slabs and stairs are used in the Bukkit/Spigot plugin Epic World Generator and they look pretty sweet. If it's an option in the RTG config, then everyone wins.

There isn't a version of Forge for 1.9.2, how could there be any Forge mods for 1.9.2?
