Step 1. Create a folder, name it whatever you like. This is going to hold ALL the stuff you need for your pack.
Step 2. Go to %appdata%\roaming\.minecraft then open bin. Extract minecraft.jar to your folder created in Step 1.
Step 3. Now is the fun part. Go through the minecraft.jar's extracted contents and replace everything you want in manually like we used to do in the .jar in Winrar/7zip.
By doing this, you can mix'n'match your options(which is what I've done) and add in items from one pack, and texture from another.
Step 4. Go into your folder, select everything including all the crap that's there, and right click > add to archive.
Step 5. Name your archive custom and change the option to a .zip
Step 6. Copy + Paste the .zip to %AppData%Roaming\.minecraft\texturepacks
Step 7. Open Minecraft and go to Mods/Textures option, and select your pack.
Step 8. Click Done and play.
I didn't have to use the patcher at all, but it all worked for me.
