I'm a little confused and need your advise! My goal is to enchant boots with 5 max level enchants in the following order:
Unbreaking III
Protection IV
Feather Falling IV
Depth Strider III
Mending
To be aware of wasting the enchanted books i went to a local test world, set the gamemode to creative, added an anvil, diamond boots and the above listed enchantments boots to my inventory. Then i got myself some levels and moved back to survival gamemode. Prepared with 150 levels to avoid annoying mode switching I started to enchant the boots by combining the 5 books and then put that one on it without any problems!!
So far, so good! I logged onto the server I am acutally playing on and went to the xp farm. 78 levels later I started to combine the enchanted books, beginning with unbreaking, then protection, feather falling, depth strider and mending. Stop, no mending, because its to expenive??? Ok, maybe it should be better to put the books apart from each other on the boots? No, sorry! Same result, mending is again to expesive!
Is there a difference between smp and single player? In single player it works. Tested it on 3 other maps!

Edit: Now I'm totally confused! I did another test by using the books apart in order of enchantment weight, beginning with the lowest. This works! Anyhow, an answer to my question would be appreciated!
The main difference is between Creative mode and Survival.
In Creative enchanting and using the anvil have no level cost and, basically, no limit on how many times you can use an anvil on an item.
In Survival the cost of using the anvil goes up every time it is used on the same object and there is a hard limit at 6 or 7 times (that's when it says "Too Expensive")
So you need to be tactical when combining items on the anvil.
The trick to adding a bunch of enchantments is to imagine drawing a pyramid shaped graph of the combinations, including any combinations done to get the books in the first place, and keeping it as wide and low as possible.
Assuming all the books came directly from the enchanting table, treasure chests or villager trading, all of them would be one level (pyramid level, not enchantment level).
Combining 2 items gives you an item that is 1 level higher than the original item with the highest level.
Adding 2 level one books together gives you a level two book, adding another book would give you a level three book add a fourth and get a level four book.
If, on the other hand you combine the 4 books 2 by 2 you get 2 level two books and combining those gives you the same book as in the first example but this time it's only level three.
So the first example has a high and skinny pyramid and the second one is low and wide.

My guess as to why your final example worked when the previous didn't is that some of your books had already been combined on the anvil, for example combining 2 Protection III books to get Protection IV?
In that case starting with that book would have used up more "Pyramid levels" since you'd be starting at level two instead of level one.
If that book were added later in the process its extra level wouldn't matter since it would be the same or lower in level as the book or boots it was combined with.

Yes, using an anvil to combine 2 items that share an enchantment will give the next tier of that enchantment if there is one... but only when the 2 items have the same level of the enchantment. Level 1 + Lv1 = Lv2. Lv2+Lv1=Lv2. Lv2+Lv2=Lv3. Lv3+Lv1=Lv3. Lv3+Lv2=Lv3. Lv3+Lv3=Lv4. Et cetera.
In other words, if the 2 items have a same-level shared enchantment, it will be upgraded. If not, the highest-level one of the two will be used.
Yes, your example matches my first try, maybe even with more previous combing! At the second try wich worked I got the Protection IV book from trading with a villager, but Feather Falling IV came from two level 3 books which i got from trading. Whatever, extending my villager trading is now much higher in priority!
I already was thinking about that the Items have a limited capacity, but that will be more filled with combined book was not part of thinking! This will be tested as soon as I got a Looting III villager!
At rework count 5, the level penalty for rework all by itself is 31 levels.
At rework count 6, you can't combine things any more.
If the Feather Falling book had already been reworked from two lower-level Feather Falling books, and you had applied it first to the boots, then you would be stuck.
Basically you always want to arrange things so that you're combining two things with the same or similar rework penalty, if you can.
And you want to add most expensive enchantment to the item first.
Anvil cost is rework penalty of either the target or the donor (whichever is higher) plus overall cost of enchantments being added to the target. If that total cost is higher than 39, you can't do it. Looking at wiki, cost of adding all 5 enchants:
Mending 4
Unbreaking 6
Depth Strider 12
Feather Falling 8
Protection 4
Total 34 in its own. If book has been on anvil 3 times, rework penalty is 7 so total 41. No go.

yes, every time you combine 2 enchantments, you get a 'rework count' on the result. (1 + the highest rework count of the ingredients.)
The game won't allow the rework count to get higher than a certain amount - 6 or 7
So doing it right:
Mending on Boots
Unbreaking & Protection together on a Book
Depth Strider & Feather Falling together on a Book
Resulting 'rework count' = 1 max
Boots w Mending + Unbreaking & Protection Book
Resulting 'rework count' = 2 max
Boots w Mending, Unbreaking, Protection + Depth Strider & Feather Falling Book
Resulting 'rework count' = 3 (Extra levels needed ('level penalty') = 7)
Doing it WRONG:
Boots + Mending = rework count 1
Boots w Mending + Unbreaking book = rework count 2
Boots w Mending, Unbreaking + Protection book = rework count 3
Boots w Mending, Unbreaking, Protection + Depth Strider = rework count 4
Boots w Mending, Unbreaking, Protection, Depth Strider + Feather Falling = rework count 5

You should be able to add books one by one too
Boots + DS 12
Boots + FF 9 (rework 1)
Boots + U 9 (rework 3)
Boots + P 11 (rework 7)
Boots + M 19 (rework 15)
But total is now 60.
