That said, nothing in Vanilla holds a candle to what modded offers. The Betweenlands is probably still the king of creepy for me, but The Midnight (it's a dimension where your portals don't form matching pairs, so going in is a one-way affair and you have to wander the dimension to find another) is close behind now.
The only times I feel apprehensive are when going to the Nether for the first time and the End, due to the possibility of a bad portal spawn (always bring some Ender pearls); as far as mineshafts go, I explore them on a near-daily basis so they are an entirely normal encounter. I also feel a bit nervous when exploring the top of a large ravine, especially the ones in my modded worlds (in one case they got so deep that falling to the bottom was certain death, even with maxed-out fall protection. There's also enormous caves with lava lakes so large there is no chance of reaching any side without Fire Resistance if you fell in the middle, but that is why I always pour water on any lava I see).
For some reason (Bad Karma maybe?) my end portal always seems to end up on an itty-bitty platform way out over the void and the dragon generally kills me before I even get to the land. A real pain. I'm not fond of gigantic lava falls in the nether either.

I still haven't been to the End, so I guess I'll have to go with that... More than anything else, it's the idea of the endless fall into the Void, I think. (I also tend not to spend much time in the nether; i recently started a new world - the first "real" world i've started since Clerics stopped selling eyes of ender, and while i'm kind of dreading having to go and set up a new nether base the lack of an ender chest (never mind potions) is starting to become a problem...)
I have arachnophobia, so abandoned mineshafts terrify me. The big spiders I can handle. Cave spiders? No no no, I'd rather fight two Withers at once than stare at one of those.

Come to think of it, I mainly play fighting-based games.
When making minecart tunnels in the Nether, and extending tunnels over lava lakes (I tend to build quite high) one block at a time ... its nervous enough just placing the next block, but to suddenly hear a Ghast followed by a fireball attack can make me things really jittery ...
The scariest places for me are Caves and any other place where creepers can sneak up on me. Abandoned mines are much better for me because unexplored tunnels can be easily blocked off with fencing obtained in the mine. I don’t mind the spiders.

I agree about using the fencing to block off areas, and I also light up all the tunnels as I clear them. Easy to do as you always get enough boards and coal to make as many torches as you need.
You get so much fencing that the only thing I find a use for most of it is as fuel.
I don't bother collecting fences or wood from mineshafts - a single stack of logs allows you to make 2048 torches, which is usually enough for 2 play sessions (yes, usually; in my modded worlds with a double chest size Ender chest I bring two stacks with me); the rails and cobwebs I collect are already so much that I modded in a couple blocks so I can compact them like you can with minerals (9 rails = 1 rail block; 4 cobwebs = 1 cobweb block. Prior to using Silk Touch shears I just crafted string into wool (hence 4 cobwebs per block), in either case I only collect cobwebs from cave spider spawners):
This was while exploring an enormous complex of mineshafts in my first world (vanilla generation), with more than 1000 rails collected on two days in a row, plus more not shown. Most of the 17 spawners I found on these two days were cave spider spawners (based on the moss stone in an average dungeon I found 3 dungeons and 14 cave spider spawners):
This was my temporary storage chest after I finished exploring them, with more than 16000 resources collected (without Fortune), including 2739 rails and 985 cobwebs (analyzing the area in MCEdit revealed another 21 rails, 1542 cobwebs, 14450 wood planks, and 9692 fences, plus a total air volume of 402776 blocks and 3289 torches, all of which I placed as I removed them from natural generation so MCMap doesn't show them until I've explored them):
