Reason of inconvenience: I was playing hardcore and had just defeated the ender dragon. I had forgotten that my spawn was directly in front of a lava pool.
Inventory: Enchanted Sword, Enchanted Armor, 2 Enchanted Picks
Cause of death: 6 Skeletons and one spider jockey jumped me wile AFK.
Reason of inconvenience: Skeleton took all my stuff.

Cause of death: Fell in lava.
Reason of inconvenience: Me and my brother are making a protected bridge in the Nether that leads to the Nether fortress since it's 400 blocks away across the lava ocean. When I went through the Nether portal to get the newly made glass from the furnaces at home, a Ghast shot it's fireball at the portal and deactivated it cause we didn't think we need to protect it. When I came back to the Nether with like a few stacks of glass, a new portal spawned on our bridge, I walked straight out of the portal and fell into the lava pool. Collecting all that sand to turn into glass for nothing.

Blew up my home(I live in a tiny ass 5x5 dungeon)
To this day, I have no [place generic rage here]ing idea what happened, and there were no creepers, I was on peaceful(world's biggest wuss 2011)

Inventory: Atleast 5 or 6 stacks of End stone, Dragon Egg, full enchanted diamond, sword of looting and power, bow of infinity.
Cause of Death: I just had defeated the end dragon, and I starting gathering end stone for building projects. Then suddenly a random hole appears and I fall into the void.
Cause of death: Standing a few blocks away from a lava pool I noticed a diamond ore block above me, standing off to the side because I know better then to mine right over my head, Mine the block, must have been a cave system over me because after I mined the block an FING CREEPER FALLS THROUGH THE HOLE!! Blows up before I could get my sword out throwing me into the lava pool... lost everything... Suck.
Inventory: Diamonds, Iron Ore, Gold Ore (basically a 3 hour cave crawl's worth of stuff)
Cause of death: While doing my usual cave crawls in a mysterious yet vast cave system i had found, i did not notice my health was down to 3 hearts. I looked up to dig out some smooth stone and was suffocated by gravel
Inconvenience: Lost all my stuff and had no hope of finding my way to a spot in what seemed to be a never ending cave.
Lesson learned: Digging up is bad ju ju.

Inventory: My first ever silk-touch pick, plus a lot of other good stuff I should never have had on me for this task, but no water bucket.
Cause of death: Hollowing out space for the business end of my cave spider farm, which happened to be below lava level, and broke a block above head level. Lava came pouring out, and I was too slow to get a block back in place. (Only part of this is due to incompetence, mind you -- my computer is very bad, and lava suddenly appearing and flowing makes it lag worse than anything else.) Being in a hole already, I had nowhere to go.
Inconvenience: My pick!!! Plus, with the hole half filled with lava and the source block hard to get to being underneath a bunch of things I'd already built, the mess took a long time to clean up.
Lesson learned: I wish I could say "don't carry your best stuff when you're doing something dangerous that doesn't require it", but I still do that. I wish I could say "carry a water bucket on your hot bar at all times", but I don't do that. "Always use fire resistance around lava?" I have no shortage of blaze rods or slime balls, but nope. "Play with the sound on at least, you idiot?" Nope again. I have learned to dig a 1x1 hole below the level of my feet for lava to path into when knowingly working below it. And I have learned to gravel all the things -- even the inconvenient bits -- when preparing to actually build inside a former lava lake, because you never know when you're going to go one block too far and kill yourself, otherwise.

Cause of death: Blazing in the Nether, got enough rods to complete alchemy room and make some fire resist pots when I decided to get some wither skulls on my way out. I went after the lone bow skelly to get rid of him, didn't see the wither skelly sneak up behind me. Got "withered" but didn't realize it, and once I did I started running only to have a blaze fireball me. Died close to my portal, but I guess the fireballs caught my sword and legs because they were gone when I got back.
Lesson Learned: Don't go charging into battle, and once you're mission is complete, don't stick around unless you already have the necessary equipment to continue. Also, always bring a bow.

I decided to put all my stuff in my chest.
"Why won't my chest open? o_O?"
Suddenly my floor starts randomly disappearing. Blocks are vanishing for no apparent reason. Unfortunately, I was so smart I built my house over a freaking ravine, and the block I was standing on decided to go on vacation.

When you dig into space, you get me, a galactic shovel. When you dig into the ground, you get an overworld potato.
Having diamond armor and diamond sword with every enchant, I fianlly made my nether portal. feeling ready, i went in and as i walk out of the portal, i fell off the portal into the lava down below. Inconvience: having that sword with every enchant possible.
Inventory:Full diamond armor with protection 4 (boots featherfalling 4, helm aqua affinity+respiration) Stacks of ore from a 4.5 hour caving run, and a nether star from a wither boss expirment Cause of death: Shot into a ravine, had .5 heart. then a Mr. Ninja Creeper dropped on my head. Reason of inconvenience: Although the ravine was where my storage room is, most of the items splattered into unexplored caves and I lost about 4 stacks of Iron and a nether star

Creepers gonna creep....have to get all my exp back and some of the somewhat rare materials were lost....only a few, but still.
