I find them by following suspects around inviisbly using vanishnopacket, or using a logging plugin to check griefings while i was offline then once i catch them i freeze them, demote them, revoking build rights, and rollback in front of them. After that, my admins and I use a variety of plugins to just harass them by rocketing, slapping, smiting, barraging, tping into the nether over lava , spawning 20 cave spiders/creepers at their location, etc. until they leave, then finally I ban and IP ban them.
I will use LogBlock as well as following people around to see what's happening. I also use MCBans to see if people had previous bans, and I have a whitelist to prevent bots from getting on.

Interesting I didn't know such a site existed... I also have a whitelist as well. Is log block a plugin for bukkit or something?

Also, with bukkit plugins you are able to log, prevent, stop spam, and keep references.
