Chances are you did it wrong. Make sure TCP 25565 is forwarded to your LAN address, and give people who you want to connect your WAN address.

I'd check a few things:
1. Do you have a static ip? Create one and put this into your server ip in the server.properties file.
2. Do you have a firewall? Make sure your port is unblocked.
3. Do you have more than one firewall for some reason? (I'm asking because it's happened to me in your situation)

Slight issue I've found with the new way /give works. It's stacking tools in the same block in inventory, which is in turn clearing any existing tool's durability. Would it be possible to modify this for tools so that it first searches for an empty inventory space, and drops it if none are available?

Double-NATing is when you have a router or a modem that acts like a router in front of another router.
It's basically giving the inside router an IP address and the inside router is giving IP addresses to clients, instead of the actual modem giving the WAN IP to the router and letting it handle NAT.
