Optifine's F3 Debug screen displays two numbers for FPS. The first number shows average FPS. The second shows your lowest FPS. The second number constantly fluctuates drastically, going from 160 fps (I have a 160Hz monitor) down to as low as 20 fps. Every single time this fps drop occurs, there is a very noticeable visual stutter that lasts for a second or two. This happens repeatedly every few seconds. It happens when moving or standing completely still; there's no difference. It doesn't make the game unplayable, but it's very very annoying. The game runs smoothly for a few seconds, then stutter, smooth a few seconds, stutter, etc. etc.
I've searched the internet for answers and advice that others may have received for a similar issue. I've tried turning on and off Vsync, as well as Gsync, with no effect on the FPS fluctuations. I've tried all sorts of different settings configurations to no avail. I use Optifine, if that wasn't already obvious. I've tried allocating more RAM; tried going back to 1G default; neither remedies my issue in any noticeable way. I've cleaned out my PC for dust. It's been in a repair shop not long ago for the same exact issue, which wasn't resolved; was told my PC was running everything "smooth as butter". Total waste of money. I'm not too sure what to try from this point, aside from getting an entirely new PC, which I don't have the money for and shouldn't need considering I've upgraded my current PC to a point that should be crushing Minecraft.
As a side note, I'd like to mention that I experience a similar stutter when playing Diablo 3. I'm not certain if it's something else causing that issue, or if the stutter in both games is linked to the same source.
Any help or tips would be greatly appreciated.

Generally you get no responses when people don't know what's wrong.
V-sync is turned on in the screenshot you posted; this, combined with FPS drops under 160, could half your framerate any time you're under half of your current framerate (E.G If you drop under 160, it'll go to 80 FPS, if you're under 80, it'll go to 40 and so on).
As for optifine, out of personal experience, I've not gotten huge benefits from it. I'd suggest doing your testing with vanilla since, as you said, your PC should shred it anyway.
Other than that, i'd think chunk loading, but since it happens as you're standing still I'm not too sure about it. Are you on a harddrive or SSD?
I doubt that it is an overheating issue, but to make sure, you could keep Speccy open on the side while running Minecraft, and switch to that whenever you get a drop.

If V-sync is off, the exact same thing happens. It isn't a V-sync issue as far as I can tell.
I have gotten benefits from using Optifine. The stutter is even worse without Optifine. I've tried running the game "vanilla" and it's the same deal.
I have the game on my SSD.
I've kept an eye on temps while playing and nothing overheats.
Also, I understand that people generally don't respond if they don't know the answer. I figured after almost 2 weeks, someone would have at least some sort of insight. Thank you for replying.
