Okay. So there's multiple problems that have arose with my new windows 10 computer. None of these things happened with my old computer which was 30x worse than my new one. The mods I'm using are the same ones I used previously and I haven't had problems with any of the mods before. I'll show a recent crash log and also list off my problems, if any more information is needed I'll gladly respond.
Problems:
1. When the &k color code is in chat, at first it does its normal function then the letters become solid. Before a re-download of Minecraft a while back, &k would simply crash my game after around 10 seconds. After re-downloading, the aforementioned problem occurs of it appearing as normal for a few seconds then it going to normal scrambled letters that aren't moving around anymore.
2. Whenever I type / in chat and then press tab (shows all available commands, and because of there being a lot of commands on Hypixel + admin commands showing, there's a considerable loading time between tabbing and it actually showing what you want) my game crashes after less than 20 seconds. This is a problem because I don't remember most of the commands that come with my mods to customize them, so I need to search through the tab list of commands to find those.
3. The game is really laggy at times, despite this computer being a lot better than my old one. Whenever I'm on Hypixel, my CPU usage can reach up to 96%. I have low settings on and still get lower FPS than my old computer. Do note that my old computer was Windows 7 and was just a cheap shitty quality one I was using temporarily. I didn't transfer any data from there to this computer, everything is freshly downloaded on this computer.
The only mods I used are for convenience, such as Level Head mod that displays peoples levels above their head. None of the mods I use are malicious or ban-able, and in my 1-2 years of using practically the same mods on my old computer I never experienced these problems. Please don't just comment that its because of my mods, if you do STRONGLY believe that, then make sure it actually makes sense. Crash log is attached below.

Sorry the crash report is formatted weirdly, when I copied it all in notepad it removed all the spacing between the first part of the crash log and made it into one big mess of words, and I didn't know what to do to prevent it. *It* doesn't like something on your system. At first I thought your Java version 1.8.0_51 was pretty old, until I saw you are running Minecraft 1.8.9. If you still have access to your old computer, I would compare your OpenGL versions.
Have you disabled your mods to see if it stops crashing?

I believe it is your mods because that is exactly where the crash is.
*It* doesn't like something on your system. At first I thought your Java version 1.8.0_51 was pretty old, until I saw you are running Minecraft 1.8.9. If you still have access to your old computer, I would compare your OpenGL versions.
