I keep getting lag spikes every few seconds at random intervals. I've tried increasing ram, turning off vsync, removing the resource pack, and toggling some of the mods that I thought might be the culprit. For some reason every now and then the game will start without sound and the lag spikes wont happen, but using F3+T to reload resources brings back both the sound and lag spikes.
Edit: It definitely appears to be sound related as on the debug pie chart it suddenly gets engulfed by yellow every time the game freezes. However, putting all of the sound sliders to zero doesn't seem to help.
try getting fastcraft or a similar fps stabilizing mod. It helps a lot. Also DEFINITELY get optifine, it is a FPS lifesaver. Check to see if you have chunk updates or something too high, it sounds like your game is having trouble loading new chunks. Check your render distance. Also, I find that putting the memory limit on like 50 helps, vsync is terrible.
I already have optifine and tried installing better fps, neither worked. Messing with my render distance didn't help at all and I don't think its chunk loading as the game runs at a solid 60 frame rate when there's no sound. Thanks anyway.

try changing your mipmap levels...? Also turn on fast math and other performance stabilizers?
It turns out that avast antivirus running in the background was causing it. After uninstalling it and playing for a few hours I still haven't had any lag.
