I've tried everything that I could find here, and everywhere else so far, and nothing has made any impact at all. No texture pack/with texture pack, the results are the same. 1gb/4gb allocated, results are exactly the same. Any insight would be greatly appreciated.

I was chatting with some folks who seemed to have this exact same problem after the creator update. They all told me to simply uninstall it, that fixed all of their problems. I'm a bit hesitant to do that, but it might be what I have to do eventually.

I'm already using the most recent AMD drivers supported for the card. I'll give soccer's tips a try.
Edit: Not seeing a place to reset defaults in the catalyst control panel. Where abouts is the defaults for pointing java at the amd graphics.

Use the Forge Profile . First customize it by going to the Launcher tab . Enable Advanced setting . Select the the Forge 1.12.2 profile . Enable Java Executable and JVM arguments . Browse to the Oracle Java directory , example C:\Program Files\Java\jre1.8.0_162\bin\javaw.exe and set JVM to -Xmx1G etc......
I've never used mods before, and I don't have a /mods folder, so I suppose I can just create it, then a sub folder labeled 1.12.2 and then put the 2 jars in there? And it'll pick them up automatically?

Is this a laptop or Desktop ? If the system is a laptop it will have dual graphics and a control panel setting to control the soft-switch , it must be set on use HD5870M + ???? , use globally
Windows Power Options must be set to high for games ,
Your problems all stem from how you have configured your system
JVM arguments set at default -Xmx1G -XX:+UseConcMarkSweepGC -XX:+CMSIncrementalMode -XX:-UseAdaptiveSizePolicy -Xmn128M
Then go into Minecraft Video settings ,
Optifines options ,
Render Distance:8 Nornal
Graphics to FAST
everything to FAST
Max Framrate: Unlimited
Play with the settings . you only have 1 Gig for frame buffers so turn down screen
resolution 800 x 600
Kill background process from Task Manager
All this information is already available by searching and just reading .
Try a Linux Mint 18.3 Live USB OS and test Minecraft in Linux to compare with windows

Are you sure you didn't lock your FPS to 15, or your monitor's refresh rate to 15? I find it unlikely that even after trying all those steps, it's still at the same FPS. The only possible issue would be a locked FPS. If your monitor refresh rate was somehow set to 15, then that explains why you're getting 15fps with VSYNC on.
Make sure your FPS is set to Unlimited in your Minecraft Settings. Toggle off VSYNC.
Refresh is 60 shutting off vsync, and setting to unlimited helped a tiny bit, it's up to about 20ish or so. Which is enough to actually deal with. Thanks for the help. I'll run with it like it is now, and see how much it hurts when the dragon breathes next time I do it.

This is your problem.
Xmx is the maximum amount of RAM you want to allocate.
Xms is the minimum.
Your Operating System (Windows) also needs RAM to function, you can't just give Minecraft half of that RAM.
Delete -Xms4G and just use -Xmx2G

Your GPU only has 1GB memory. If you have big textures such as 32x or higher, it will use it up a lot faster, giving you less FPS.

Legacy Cards don't support some new grapic features
Most current AMD drivers https://support.amd.com/en-us/download/desktop/legacy?product=legacy3&os=Windows 10 - 64 . Try the Beta
Optifine and Foam go in .minecraft / mods / 1.12.2 / , directory
Read the instructions for the Crimson beta driver , this is as good as it gets for your Graphics card .

Download Speccy, File > Publish Snapshot. This would give a lot more information on your system such as processes, and etc that you're running.
