Please make a thread in the modded support section as this is a different issue relating to mods and not the vanilla game.
But it maybe just the fact your pc is too old as it's below the requirements for minecraft .

Your best bet would be to install the Catalyst Software suite from this page https://support.amd.com/en-us/download/desktop/legacy?product=Legacy1&os=Windows Vista - 64
But that hardware is considerably old and not really enough to properly support the game.
