I have tried removing Inventory Tweaks and Optifine, but that has not done anything.
That is intended behavior. The Crafting Station from tinker's construct holds items even after exiting the gui, it is superior to the vanilla crafting table because of that.
Since you're a fellow mod author, I thought you might want to inspect the Crafting Station's class for yourself here. As you can see, it extends BlockInventory.
By that, I mean that even after I craft the item, the items stay in the crafting station.
My guess would be some sort of server/client desync. Looking at your modlist, the only mods I can see that could cause this are InventoryTweaks, MineTweaker, or one of the 16 coremods installed.
If you're getting a bunch of lag spikes or "can't keep up, is the server overloaded" messages, I would lower the amount of ram allocated to minecraft to 4-5 gigs, 7 gigs can cause spikes when java goes to garbage collect.

Here is the debug info in the log along with a list of mods from another crash (server tick loop): https://hastebin.com/fijelawoxo.sql
