I don't see any particular mod that's causing an issue, however Optifine does modify the options menu, try removing it to see if it fixes the issue. If it does fix the issue, then Optifine is conflicting with another mod.

Simply removing the optifine .jar from "mods" didn't work. Do I have to do anything else to make sure it's removed? If not, then I guess it's not OptiFine that is the culprit.

I went through all my mods, taking out 1 at a time. Turns out the "MoreCommands" that messed it up. Not sure if it's a config error within the mod or if it's a conflict error but at least I can enter my controls now.
