Graphics driver issue looks like, whats your specs such as MC Version, Forge, any mods, lastest log would be handy, and your pc specs like cpu, gpu, driver versions for gpu. You can get all the non Minecraft info from running dxDiag in Run.

Take a look at this, I can't play any modded version of minecraft because of this stupid bug that no one has a fix for somehow.
