Game Version 1.10.2

So, I have a personal mod-pack comprised of mods for performance, visuals, and gameplay overhauls, and recently, it stopped working.
I can't figure out which mod it is that causes it, but I know its a mod because I uninstalled it,
re-installed, and played on a clean save and it was fine.

My problem, is a glitch.

This glitch make the game completely un-playable, as it makes the camera flick away, and forces the chunks to unload, upon movement.

These stills, are what is seen when you stand still, however, as I stated above, the chunks unload when you try to move.
You can pan the camera all over but, that's as far as you can go.
Below is the manual crash report. Hopefully it can tell someone what it is that's causing it.

I have about 36-40 mods active at a time, so it's a bit frustrating, to say the least.

So, I had to trigger it manually because the load-out runs, and for all intent and purposes, it's playable, except, that when you move,
the game unloads All the loaded chunks in a blink of an eye.

My Render distance is set to 8.

I've been running the latest version of OptiFine.
