Also, the day my game started crashing, I installed two different programs to my computer and deleted both of them (beleiving they are the cause) but it turned out that they weren't. They are Internet Download Manager and AVG AntiVirus. Nothing else different happened, as far as I remember.
Oh, I have a theory now! It happens in Survival mode. I fall from the same distance in creative but nothing happened. Tried the same thing on the same map in survival and it crached. Here it is:

Can you try and see if it happens on singleplayer, if so post a log of that too.
Oh, I have a theory now! It happens in Survival mode. I fall from the same distance in creative but nothing happened. Tried the same thing on the same map in survival and it crached. Here it is:
