I have win 8.1, and should have the newest version of minecraft seeing as I just re-downloaded it to try and fix this.
V-sync and VBO are on, have always been on, haven't changed a thing. I've even turned it off, saved, and turned it back on. No good. I also have it set to launch with my NVIDIA GPU rather than intel.

Edit: I tentatively solved my own problem. Even though I swear I've tried it before, reinstalling Java and restarting it several times after that did the job.
This is Minecraft alternate downloads page. I would use the second Windows platform installer/launcher since it uses your installed Java version rather than a bundled version which is OLD. Here is an older topic where gerbil explained it.
