i made a video about this because i dunno how else to really explain it
taking damage underwater seems to cause the whole world to derender, i have no idea what's causing it
any suggestions would be appreciated. as always, i can provide additional information if its needed

Couple of common questions:
Are you using Optifine or Fastcraft?
Does this happen on a new world?
Have you tried creating a new (test) world with fresh configs (everything in default state)?
Have you tried removing mods 5-10 at a time to see when the problem goes away? (Would also suggest trying this step on a test world as well.)
If you're using Optifine, I know that Fast Render turned on can cause a lot of strange issues. So, try turning that off, if it's an option for you.
While I can't produce this problem myself obviously, I'm just throwing some things out there that maybe you haven't though of yet. Do you have a mod list you can provide? That might help others in identifying if they've seen this particular issue with a mod in your list.

We have restarted the world once after this had been affecting one player, which is the current world we're on. The config files arent really clean though. We didnt really mess with many of them though, and the ones we did shouldnt have anything to do with rendering. i suppose it could be one, but i dont remember everything that has been changed, so i'd rather not wipe them all.
we do have optifine, but i already had fast render turned off, since it was actually causing a crash to desktop before for me lol

one thing that i did think about after making the video, and noticing what happened in it. i tried disabling fog completely, and it seems to have stopped the bug from happening. so i think it IS tied to the fog somehow, and isnt actually anything with the world rendering.
i really dont feel like taking out a few mods at a time until i go through all of them to figure out what's causing it. if it was only a few mods in here, like 50ish, maybe. but there's about 150ish in here, because of how my friend initially designed the pack. takes a long time to boot, so removing a few mods at a time ends up being a half hour per check instead of like 5 minutes. sorry

Yeah, I understand the huge modpack thing, that's what I'm dealing with setting up right now, but sometimes you gotta do what you gotta do.
As for the configs, just make a back up of them somewhere else, or rename the current config folder to "config1" or something like that (do the same with your world folder, so you don't lose that either), so that it loads a fresh set of config files, then delete it when you're done testing. You would be surprised at what can affect things, even if it seems like it shouldn't. For example, I recently had witches spawning on top of spiders, every time. There was not a single witch that wasn't riding a spider. It turned out to be a config setting for zombies spawning w/weapons from another mod, with that config option set to "0.0" instead of "1.0" for 100%. For some reason, that null value caused some other random problem to come up. So, moral of the story here is, give it a shot. You might be surprised.
Regarding Optifine and the fog, did you try changing how the fog is rendered too? (like fast, fancy, distance, etc) I don't think it's Optifine at this point, since you said turning the fog off fixed the issue, but hey, it's worth a try.

this made me realize how i can clear out all the unused config files without messing them up. i tried doing it, and now there's not any unused configs in the folder. so far it seems to have fixed it, i really dunno what caused it still though.
because i have access to the server, i took my clientside configs, deleted them, regenerated them, and then moved the server's configs over to the client. when prompted about overwriting files, i chose to skip. that left only the proper configs in the server folder. delete all the client configs again, copy the refined server ones in, and done.
hopefully it really is fixed now and it isnt just that it happens to not work at the moment. ill find out for sure later when my friends begin playing lol
