It seems the answer for these is always different. I can't help but think, "oh, they've fiddled with the settings and have not toggled them back." Well both Optifine and the Nvidia control panels have reset buttons now.
Few suggestions:
Set Vsync, AF and AA OFF, preferably within the MC and Optifine options.
In the control panel, set "threaded optimization" to AUTO. It can be set per App instead of Global.
videos online say that i should be running 60-80 fps on minecraft maxed out shaders. but i only have between 23-30 fps

i have tweaked with the minecraft, optifine, and nvidia settings extensivly
i have also googled the crap out of this issue with no solutions working for me

its only an issue with shaders. without it i get 200-600 fps
