Minecraft saves your server information in a file called servers.dat, located in your .minecraft folder (or the Apple alternative). Deleting or moving that file will reset your server ip's.
As to what could be causing this, I have no idea. Unless you or someone else has been moving your stuff around, I don't see how that file would randomly disappear.

I am on 1.8.9 and when i check my server list it sometimes gets deleted and i have to retype all the IPs and its getting really annoying. Any help?
