Wrong place in the forum, but it looks like it MAY BE the shaders since it didn't show entirely in the preload.
EDIT: It says that the shaders are supported at the very end, but you may not have the coremod installed or installed correctly. Try reinstalling the coremod, it may fix it.

Shaders Mod 2.4.12 is incompatible with Optifine D3; please downgrade Optifine to D2. Also downgrade Forge one version from 1487 to 1486 if downgrading OptiFine alone does not solve the issue.

Downgrade Forge to 1468 or recommended 1450,
OF to D2 and
use Shaders 2.4.12
For Right Now, higher versions cause a crash with Shaders.
Uh, pretty sure shadersmod isn't a coremod.
