I reset all the settings in AMD Catalyst Control Center and in Minecraft to the Defaults. It Made the problem worse the lines are there even when I get close.
Right click desktop -> Graphics Properties -> 3D Application Settings -> Javaw -> Anti-Aliasing Mode: Use Application Settings Or Override And Select None. Do The Same With The Other Options.
Just give me a screenshot of your AMD Catalyst settings.
How do I fix this please help. My Minecraft settings: Render Distance: 16, Graphics: Fancy, Anisotropic Filtering: 16, Smooth Lighting: Maximum.

Do they go away when you get closer? This happens on mine, I never saw it as a problem. I think it's normal, not sure though.

you could try messing with mipmap levels, or possibly AA settings, but not sure if either would have an effect
Yes they go away when I get closer. I asked my friends they said they don't have this problem.
I think this is a feature of Minecraft.

How do you turn off AA, AF and MipMaps in AMD Catalyst Control Center.
I meant in AMD Catalyst Control Center. Turning it off on Minecraft does not make a difference.
So how do you permanently turn of AA in AMD CCC?. I have the latest beta version.

Nope did not work. I even installed the recommended AMD Catalyst for my graphics card(Omega 14.9) but still nothing.
