If you're having CDT (crash to desktop) when trying to start up modded minecraft and you have optifine installed, check your crash report and look for something along these lines (they'll generally be near the bottom of the crash report)

if you see anything similar to these what you want to do is remove optifine.
"But Sarge, if I do that my FPS will be unstable/unbearably low!"
Yes, that is normally the reason people install optifine, however in heavily modded clients it tends to cause crashes. But there is no reason to worry! There is a mod that will stabilize/increase your FPS just like (If not better than) Optifine!
It is called Fastcraft, a mod that was created by player (a developer for Industrial Craft 2) which does not produce these crashes yet offers better FPS, it can also be used on servers for any lag issues they may be having.
I just uninstall the mods that I can honestly tell myself I don't use. This improves my performance and reduces me from 70 to about 50 mods. It also reduces the crashes and conflicts with mods like Optifine.
