What is your render distance? It looks like normal Minecraft loading/unloading. It only loads the chunks in your immediate area....and if your render distance is small it isn't very many chunks. It unloads to keep the lag down.

I've had this problem for a couple of years now and still haven't found a fix.
Chunks that I constant am around and have loaded vanish and reappear constantly.
Here's a video showing the problem.
It's a render distance problem but I'm running on pre 1.8 with no VBOs option.
