My Minecraft has kept crashing and I don't know whats the problem here.

I think you have the same problem as me. If you use optifine it can maybe be the scenario. Optifine is interrupting with something, it is a bug, but i guess they are trying develope it better. I recomend deleting optifine or using an older version of optifine until a new version comes out
If you don't have optifine try updating drivers. updating java or playing an older version of minecraft.

try to update your graphics card.

That happened to me but it was fixed by restarting the computer after updating my GPU.
Here's the way: Update your graphics card and then restart the computer to make minecraft unlikely to crash.
I updated the graphics card and restarted my PC. Still crashes though.
