This only happens on this particular computer. This doesn't happen to any of my friends when we use my Bukkit server together. Does this have something to do with the graphics card?
Let me know if there's anything I can do, or if you've had the same problem.
You might find a little help from Optifine's configuration, but that might be too easy.
That's definitely a graphics card problem. Are your drivers up to date? Also, just to see if it isn't just you, uninstall all your mods or make a new profile and play vanilla for a little while, just to see if it isn't one of your mods.
This was all while I was playing vanilla, so I'm sure it isn't my mods.

This is a fairly old computer, and I have been looking (years) for the driver that isn't up to date which is causing this, and still no luck. They're mostly virus download pages and spam. I'm looking for ATI Radeon graphics.
Another little bonus, I recommend RadeonPro to anyone with ATI Radeon cards. Catalyst Control Center and RadeonPro should be working together and both be installed on the computer. RadeonPro is basically a more configurable Catalyst Control Center with profiles. Learn more about it here.
One last thing, if your card is very old, be careful with the newest drivers. Normally it wouldn't matter, but there is technically a chance that the drivers are not compatible with the older cards. I would work my way up versions, find out what you have now and go up from there.
I always use the official launcher.This also has nothing to do with the launcher.
I run Windows XP, and can't find a version of RadeonPro that supports a Windows version earlier than Vista. Shoot.
I just wanted to mention since I started using this computer, some chunks have turned into camoflage-coloured cubes.
