Go to where Minecraft is installed and delete everything exept the saves folder (becuse that's where your saved games are). Then go to Minecraft.exe (redownload from here if you need it) and let it reinstall Minecraft from scratch.

I assume for the fact that the user had provided a DxDiag log, albeit incomplete, is because the game is crashing of the "pixel format not accelerated" error.

When minecraft suffers a pixel not formatted error it directs you to a website with a guide on how to update drivers, and make a dxdiag.
The op tried to supply a dxdiag (albeit partial) which tells us thats what the initial error was.
Also we are just used to this error showing up
