(1st post whoo....)
After spending days and days trying to mod and finding other guides confusing, I finally was able to mod my minecraft through trial and error. Thinking that other people should not suffer as how I did, I decided to make a forums account and create this guide:
Step 1: Research
Find what essential mods are needed. These are usually Forge, ModLoader, and PlayerAPI. Optifine may be required for others. If you are using forge (with mc version 1.3.2 or higher), DO NOT DOWNLOAD MODLOADER. You will black screen and rage over it. Or, mods won't load properly. Forge has a built in modloader so it should be fine. I prefer Forge as it has more capabilities but it is just my opinion. If the mod requires Optifine, download Forge or Modloader FIRST (not sure about PlayerAPI). In addition, use the forge/modloader version the modmaker tells you to, or else you will blackscreen. Make sure that your mod version and forge/modloader version are the same as your client.
Step 2: The Downloading
First, make sure you have anything that can open up zip files (most are in zip, few are in rar). For example, 7zip and Winrar will both get the job done.
Second, make sure you have all the essential mods and the mod itself downloaded.
Third, In your start menu, type in "run" (no " ") and then type in "%appdata%" (again, no " "). Click on your .minecraft folder. You can also press Win (windows key) + R at the same time to prompt the Run window, in that text field type in %appdata%\.minecraft
Fourth, make sure your minecraft is "clean". Clean refers to that the client is completely vanilla (original) and has not been modded yet. Later on, if you are more experienced, you can install multiple mods together at once without cleaning your minecraft. To clean it, open your minecraft, click force update, then login. Then close it when it is finished.
Fifth, BACKUP EVERYTHING. Just copy and paste the whole .minecraft folder onto your desktop so if your minecraft breaks, delete the original .minecraft folder(not contents, the whole folder) and replace it with the copied one (not the contents, the whole folder itself).
Sixth: When you are done backing up everything, open the .minecraft folder, then open the bin directory, then open the minecraft.jar with your zip opener (right click it, and select open with "name of zip opener".
Seventh: Then, delete the META-INF folder. After that, open the essential mods with the zip opener you are using, and drag all the contents in. if it prompts you with a notice, more often then not clicking yes is the way to go.
Eighth: Run minecraft so a essential folder can be created. If this works, you can create a new folder on your desktop, name it whatever you want, and copy and paste the new modified .minecraft folder. This is for if you ever need to force update your minecraft, you will not need to start from scratch. This option can also be used for step 10.
Nineth: There are two ways here (for more common mods. if there are different directions for mods such as DivineRPG and that are not listed here, follow those instead) . You can either
A) Copy and paste the zip file (not the contents) into the mods folder (or the coremods folder if specified). Sometimes this option will work and sometimes it WILL NOT. Some mods tell you to put it into the mods folder (Ex: Gravity Gun). If so, DO NOT FOLLOW OPTION B. If they only say the into the jar (explained in part B ), sometimes the mod will work in the mods folder (Ex: Rei's Minimap, Animal bikes, etc), sometimes it will crash. This is good for testing as you can automatically delete the mod from the mods folder (without have to load up the backup files).
, Using your zip opener, open the mod the same way you opened your .jar file from before. Select ALL the contents (unless specified on the mod page) and drag it into the .jar(open it with your zip opener). Again, more often then not, clicking yes if a popup appears will be the right path to take.
Tenth: Run your minecraft now. If it works, congrats, you have successfully modded minecraft. Again, you can use the option used in step 8 to back up your files. Hope you mod successfully in the future and have a good day !

Side note: Also, how do I add spoilers?
Nice. But this is quite different for us Windows 8 users since Start menu was murdered and replaced with a start hub thing.
Everything to do with "Start" is "File Explorer" now.To get to run is Windows+R now.
Thread Locked to allow it to slowly sink into the depths as it is now an outdated guide.
Since 1.6.2 the modding methods have changed.
Forge has an Auto Installer and Mods are now just placed into the MODS folder, no unzipping/file editing is needed.
