I recently installed Optifine, and am very happy with the results. There is one glitch I am experiencing that is ruining all of it for me.
When I approach an object that has a shadow, the shadow under the object disappears. Moving back away from it doesn't fix it, it's basically full-bright under the object. Moving through a dense forest is very distracting, as this flickering is happening all around me.
