This glitch right here makes me want to JIhad Mojang headquarters. So it happens in both vanilla and modded, I'm using the default texture pack. Happens in every version. EVERY VERSION. I don't want that update your Minecraft crap, it happens in 1.6, and 1.7. And once again, it's not because of a texture pack or mod. And I wait a long ass time, and it still happens, it won't go away. And here ladies, and gentlemen, here it is.

Also it just happened randomly one day, and won't go back.

Have you tried a clean minecraft install. Backup your saves, and texture packs if needed, and delete the whole Minecraft folder out of Application Support or .minecraft

Yep. 100% Everything. Mods, texture packs, every single Minecraft file, then I played a new vanilla, and it still happened.

Nope, I just don't know what went wrong!

You know what, I give up, SCREW YOU MINECRAFT I HAVE BETTER GAMES TO PLAY!
