I've recently installed Forge 1.6.4 for a Doctor Who mod and used it without a problem. However, after an attempt to modify the programming (i.e. adding my own splashes) Minecraft would crash each time I tried to start a game. It would only crash on the Forge profile.
After reinstalling both Forge and the mod, the issue with the splashes was fixed, but whenever the Doctor Who mod was in the Mods folder, Minecraft would show this crash message (I've italicized what I thought was the most important):
