Help?!? ID Mismatch

Hi Everyone.

I have tried to make a Minecraft mod pack and it runs fine with no errors.

I made a sever pack on my PC and tested it locally and once again it loaded fine.

however when I upload the server files to my Online server (Linux) it starts with no errors but as soon as I Connect i get this error on my client:

On the Server Console it says:

16:19:02 [INFO] CloudTheWolf joined the game
16:19:02 [SEVERE] Reached end of stream for /*ip hidden*
16:19:02 [INFO] CloudTheWolf lost connection: disconnect.endOfStream
16:19:02 [INFO] CloudTheWolf left the game

Any Idea why?
