When I start up Minecraft it displays the Mojang Symbol and get told to post this crash report

This its really annoying me since I've been trying to install this for about 12hours and its not working. I've followed dozens of tutorials and none of them work. After a failed attempt I start over with a new jar and this is the furthest I can get. I've had forge working before and I have about 120hours into a save with the Millenaire mod so any help would be appreciated.

How did you fix it? I am having the same problem with my minecraft forge. I'm in 1.6.2.
It happens with normal Minecraft, forge, and optifine. Please help.
