Problem with LAN since update 1.4.6 - blocks spwaning everywhere!

Ever since the 1.4.6 update, I have been having trouble playing LAN with mods. I have Forge universal for 1.4.6, Too Many Items for 1.4.6, Zans Minimap 1.4.6, and Simple Ores 1.4.6.

I can play just fine in single player mode, however I was getting some ID mismatch errors at first, but I think I fixed that problem. Now, whenever I log into my nephews LAN world this happens:

Blocks everywhere!!

Any idea what is causing this? I don't have any crash reports, just a crazy world. Also, my nephews world looks perfectly normal.

It's probably an error with Simple Ores. Force update to get a fresh minecraft.jar and reinstall your mods.

Does your nephew have the same mods?

I was thinking it might be the simple ores mod. I've already tried several reinstalls and it keeps doing the same thing. We have all the same mods except for Zans Minimap. Here is a crash report we just got:

It says Sacktrace is because of a Item.tool, probably a tool added by simple ores mod.
And remember: Before installing ANY mods, delete the meta inf.
