I'm pretty sure there is only one way to install Optifine, and that is through minecraft.jar
Try with fresh minecraft install, backup your saves of course. Deleted META-INF in the minecraft.jar then copy the contents of the Optifine zip into minecraft.jar. If problem presists, it may be because of a bad download.
