I was hoping someone could help me figure out why this happening and how to correct it.

I have duplicated the problem on both single player and multiplayer. Whenever I die, Minecraft crashes. If I attempt to rejoin the game, the game looks like it is going to load the world and then it crashes. This happens non stop and I am unable to join the game again.
