The pack I had created (Simple pack, a fixed Clay Soldiers with Optifine and Rei's Minimap)
was working not too long ago...
I removed CSM (Possible source of issue may be the Optifine Compatability Fix)
no change in error.

Curse PremiumYou might try updating Forge, this version is a bit out of date.
Rei's MiniMap can be dropped into the mods folder, though it should work in the JAR, too..
