Would anyone like a suggestion? How about we isolate if this is a hardware thing. Does it occur to anyone that no one has posted specific computer specifications? Maybe that can weed out why some people have this problem and some people don't.
I've noticed that on some servers if everyone leaves and it's just me left eventually my lag starts to dissapear. Any time there are more than 2-3 people on with me the lag comes right back.
I suggest we call this issue, "Multidisease", or "Minecancer". It's annoying as hell, and there is no current fix. There are multiple fixes, actually, but these only work for some people. So in a way, much like cancer.
Let me contribute a fix I found, which might work for some people.
If you go to minecraft.net, and go play Minecraft in your browser, it might already fix. Just go see for yourself.
If it DIDN'T fix, go open the downloaded Minecraft, while your browser is still open. Now go in multiplayer. At times, it might tell you that you had a bad login, and if does this, just wait a while before you try again (2-3 minutes). You might also wanna switch between logging in on your browser, and your downloaded Minecraft, and restart/refresh Minecraft from time to time.
The only down side from this fix, is that over time, your client is gonna lag again, and after that, it can take a really long time before you get it working again.
Mojang, please, if you read this, at least give us some clearance whether what the problem is, or that you're working on a fix. It is preposterous that we have to resort to stuff like this, just so we can play SMP normally.

[Edit]: Btw, if you try my fix, if it doesn't work for about half an hour, take a quick break. You can get REALLY frustrated by repeatedly trying. I even went and threw/broke stuff because I was so frustrated, because I had been trying for about 1 hour. In the future, I won't try for so long XD.

I'm on vista home premium SP1, 32 bit.
My graphics card is nvidia geforce 7300 se / 7200 gs, I've had problems with that and games before, some nvidia update broke stuff a while back (strange how updates intended to improve things, don't anymore)

Before two days the regular server Im playing on there was no lags.
now i can't even play in this server , i laging! and falling from the sky!
and it's like that to all the servers!
and i found out something.
i don't see my steps but my friend yes.
for example : i put a torch on the wall. after few seconds it will remove (because the lags) but my friend still see the
torch. another example, i play on note block, i hear the note block after 10 -15 seconds, but my friend hear it without delay!
1 powerhouse pc (4gb ram winxp nvidia 280 etc etc) with server and client, works flawlessly.
1 desktop pc (2gb ram winxp radeon x1950pro pentium4 3ghz), has no lag but has unplayable fps on every chunk update (if chunk update ~=0 then fps goes from 2 to 10)
1 laptop dell inspirion 1720 has serious lag but no fps problems.
Now, lag is exactly like described in previous posts - the action on server is preformed instantly, its the information from server to the laptop that is delayed. So, if i destroy a brick, it will happen instantly on server and i will pick up the brick (i can hear it from server pc) but i will only see a brick pop up like in a second or two and get it into my inv a second or two later.
The problem seems to be hardware tied (since on other PC there is no lag, in same LAN) to my laptop. And it seems that packets only from server to client are delayed, from client to server its flawless.

This lag occurs on XP with 2GB, so RAM is definitely not the issue. Not only that...

I can play on my own 1.2 server absolutely fine, but the game immediately begins to lag when I uninstall 1.2, reinstall 1.3 and join a 1.3 server hosted on the same PC that hosted the prior 1.2 game. Updating both to 1.4 does not fix this, but removing 1.3/1.4 and reinstalling 1.2 does. This is most definitely a compatibility issue with the last two updates, supposedly not software as someone has already said that the lag persisted even after a reformat and fresh Windows install.
