I play on a server most of the time, but I've also noticed this happen in single player.
The issue occurs whether I have TeamSpeak running or not; whether I have headphone/mic (3.5mm) attached or not.
I haven't noticed it in any other applications, but I'll be on the look out. It's possible it could be related to the setting in Windows 8 that allows volume to lower automatically when an incoming call occurs.
Running:
1.7.2 unmodified
Windows 8
High Definition Audio device (front jack transfer enabled, listen to mic on headphone disabled)
Driver: hdaudbus.inf (Microsoft supplied)
I believe it is a RealTek/possible clone onboard an ASUS M4A79XTD EVO mobo.
