This has only been a problem with 1.6.1. Even the 1.6.2 snapshot doesn't do the trick. I get this error every time I've tried to start the game.
Things I've tried:
-Deleting the ".minecraft" in my appdata (several times)
-Removing all traces of anything minecraft on my computer, then reinstalling
-Removing all installed versions of Java then installing the latest version
-Defragmenting
-Scanning for viruses
-Trying previous versions of the game through the new launcher
-Trying the other versions of the program for other operating systems
What does this error even mean? Is there a way to fix it as of recent or will it be officially fixed later?
Also, if there's other solutions that weren't listed above, please let me know, even if they didn't work for you. It seems everybody has a different solution at this point.
One I thing I have learned is the exit code means nothing! A bunch of people are having the same problem all with different exit codes and files that need deleted! This is a minecraft issue with a new patch or something! We are just going to have to wait til they fix it!
I didn't have D3Dgear anymore, but I knew that I had it at some point. So I re-installed it, then deleted everything it'd installed.
As it turns out, I think I still had some leftover DLL files from its installation in my system32, so I just removed those, uninstalled the program, and that got it back up an running again.

The issue was reported on the bug tracker, MCL-148.
Make sure you're not using D3Dgear.
