When I set my render distance to normal or above, around 5 minutes after, minecraft crashes. I have 64 bit java, and I am playing on 3 monitors at 5760x1080
Did you get any error report for the crash? If it's not shown on the Minecraft window itself, it is located in the crash reports folder under the .minecraft folder.
I can't find the error report in .minecraft, what folder do I look in?
There should be folder named "crash reports" or so in the .minecraft folder. the error report is a text file named "hs_err_pid_###" or something close to that.
I have also just got "The NVIDIA OpenGL driver lost connection with the display drive due to exceeding the Windows Time-Out limit and is unable to continue. The application must close.

You have a slight problem with your internet connection, or the MC servers might be down. It is trying to download the Minecraft resources (render stuff) and cant get at it. Check if your firewall is blocking Java from the internet. Windows Firewall blocks it by default.
