I've been trying to play on my girlfriend's server, but have been having some issues. After five minutes or so of gameplay my GPU usage will cut out for 15 seconds, and then work for another 15 seconds.
Specs:
GPU - Nvidia GeForce 680 (320.18 WHQL)
CPU - Core i5 2400
RAM - 16gig DDR3
OS - Win7 x64
Java - Version 7 Update 21
My rig is clearly fit to run this game at 60fps. And it does, when it's working. GPU usage goes to 10% usage, and maybe a couple degrees Celsius above idle.
I'm not entirely convinced it's a driver/graphics problem. The game becomes completely unresponsive (even in menu) when the drop to zero happens. Like the game is freezing. Otherwise, I expect, the CPU would render at at least 1-5 FPS. I might be mistaken there though.
Could be many issues why the FPS spikes and drops.

We're the only ones on the server. She has no trouble, and another roommate joining has no trouble either. Her rig has 12 gigs of RAM. So I don't think that's the issue
Secondly I would actually check the settings of the nvidia card. Usually there is an invidia icon on the bottom of the bar, to change the settings. If there isn't go to control panel>large icons(old view) then go to nvidia control panel. Tweak the 3D settings so that it's not too GPU/VPU intensive and see how that runs.
I have a GTX 680. When the game runs the usage is less than 10% of the GPU. I can't make it less GPU intensive if I tried. The only thing I force is vsync and FXAA.
Unfortunately this has been going on since the last driver update.
As I posted, I have the latest WHQL (windows certified) drivers. I have had this problem for a couple of months when I had the last WHQL drivers. Auto-update does not install beta drivers for Nvidia.
I don't think you understand. It has never been working. I've tried the last two WHQL drivers. If I go back further I'm going to start seeing drops in performance in Metro Last Light.
Well if it's never been working why didn't you return it?
I've run out of other options to help you, sorry. I think you may find better help on the nVidia forums. Clearly it seems the problem is the card.

1) What's your java version?
try installing the newest version of Java.
Also try installing 32 bit java
A double resolution texture pack that stays faithful to the original Minecraft textures.
