Yes my log is a modded minecraft. No it occurs more often on modded than regular. But it still does occur. Only difference in log is the stuff loaded in minecraft..
If it weren't for your crash report saying you had an AMD card, I'd be clueless of your problem.
Get the latest AMD drivers from here.

Read this post here this guy has the same prob It chould be a out of memory crash it says here do you have enough RAM to run minecraft with more memory? Go to system property's also here's another post I'd found You may want to update to java 7 if you haven't alredy if that doesn't work delete the .bin folder

This is not the same error... This is an error on the dll for video drivers

Is all video drivers.
100% unrelated to memory problems. I already have my ram set to optimum settings..
I read the logs. Nothing states its increasing heap sizes for memory..
Already been on latest update of java. IF you have read the logs I posted.
